Privacy notice
Effective as of
This notice explains what data this website collects, what it is used for, and who it is shared with. Protected health information created at the clinic is additionally governed by the Notice of Privacy Practices (NPP).
What we collect
Only what you type into the contact form: name, email address, phone number, the interest you select, the services you check, the language you are browsing in, and the record of your consent.
The form does not ask for symptoms, medical history, diagnoses or any free text, and the server rejects any field other than those seven. We do not collect health data through this website.
What we use it for
To reply to you, to arrange an evaluation appointment, and to keep an internal record of your request and your consent. We do not use it for targeted advertising, and we neither sell it nor pass it to third parties for commercial purposes.
Who we share it with
Form data is delivered to our patient relationship management provider, which processes it on our behalf and under our instructions, and to our web hosting provider.
Beyond that, we share information only where the law requires it.
Cookies and tracking
This site sets no advertising or tracking cookies, and its public pages make no requests to third-party services while you browse.
The embedded video and map do not load on their own: until you click to view them, no connection is made to YouTube or Google Maps. Once you do, those providers' policies apply.
How long we keep it
We keep contact data for as long as needed to handle your request and maintain the clinical or administrative relationship, and afterwards for the period applicable law requires.
Your choices
You may ask us for access to the contact data we hold about you, for its correction, or for its deletion, and you may withdraw your consent at any time. We will act on your request within a reasonable period.
Your rights over protected health information are broader and are described in the Notice of Privacy Practices.
Children
This site is not directed to children under 13 and we do not knowingly collect their data. If you believe a child has sent us information, write to us and we will delete it.
Changes to this notice
If we update this notice, we will publish the new version at this same address and change the effective date.
